Oh yea I SO need to focus and concentrate on form when I'm using a MACHINE. You know, since the machine only allows you to move 1 way, you can totally mess it up. And what is with this guy and machines? Dont tell me he hasn't figured out why people who can leg press 900 pounds can only squat 90.

that's actually not the main reason why you do it cross the bench. it's basically so that you won't fall backwards.

Oh yeah, the body should -cross- the bench, not lie the normal lengthwise. this way , you can arch the back. This will get the dumbell farther back&down, where all the good work is done on this move.

Arms should be almost straight, bent just enough to not stress elbows. Reps are a very limited range, make up for that with higher reps, 30 or so. Try 3 super-sets w/ high rep squats. Rather than adding weight , decrease time between super-sets. Add weight when you can do all 6 sets without any rest. Add very little wieght, straight arm work is mostly leverage, not iron. Sometimes you can get the effect of adding weight just by straightening your arms a little more.

Expanding the rib cage is subject to dispute. If it's possible it'll only work for guys in their early teens; the skeleton is more cartiligeneous. The common move is a dumbell pullover on flat bench, ( Nautilus pullover is better, but hard to find). Bring the dumbell back far enough to feel a good stretch, and bring up to about 45deg above horizontal. Old school guys alternate this with high rep squats, believing heavy breathing helps expansion .

Low carbs are for average Joes to lose weight. Athletes need the carbs as a source of fuel for working out. Eating those same calories as protein requires a more complex bodily process to just break down into the same energy, anyway, and protein tends to be more expensive than carbs.
